George Online LLC, and Household of Faith

“I have accepted and believe in Jesus Christ as my Savior and Lord and desire to show that in water baptism by immersion. I understand that this baptism is not equivalent to joining any church congregation and does not obligate me to become a member of any church congregation.”

Assumption and Acknowledgment of Risks AND Release of Liability Agreement

This agreement releases George Online LLC, and Household of Faith from all liability relating to injuries that may occur with WATER BAPTISM / BAPTISM BY WATER. By signing this agreement, I agree to hold George Online LLC, and Household of Faith entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

I, also acknowledge the risks involved in WATER BAPTISM / BAPTISM BY WATER. These include but are not limited to George Online LLC, and Household of Faith not being held responsible for the physical/natural or spiritual consequences (i.e. WATER IN NOSE / MOUTH, CHOKING, ANXIETY / HEARTRATE INCREASE, CRYING, DIZZINESS, BLURRED VISION, MASSIVE HEALING, INDIVIDUALHEALING, MASSIVE DEATHS, INDIVIDUAL DEATHS, ETC) of what may happen which is an act of God or bodily functions and nothing that George Online LLC, and Household of Faith has any control over, and that George Online LLC, and Household of Faith is to be immune to any lawsuits related to whatever may occur as a result of WATER BAPTISM.

I affirm that I am participating voluntarily, and that all risks are fully understood by me. By signing below, I forfeit all right to bring a suit against George Online LLC, and Household of Faith for any reason.

In return, my I will receive WATER BAPTISM/ BAPTISM BY WATER. I will also make every effort to obey typical safety precautions. I will ask for clarification when needed.

I, THEREFORE under the terms of this Agreement and sufficiency of which is hereby acknowledged, do hereby release and forever discharge George Online LLC, and Household of Faith (Hereinafter the “Releasees”) including their agents, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to anyand all injuries and damages of any and every kind, to both person and property, spiritual and natural, physical and mental, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the following:

BAPTISM BY WATER / WATER BAPTISM. I, hereby understand that; the body of water / location where the baptism may take place, may not be an area / location or condition in which I am familiar with (in the State of ARKANSAS) and that I can OPT NOT to participate in the baptism if I am not comfortable receiving WATER BAPTISM / BAPTISM BY WATER in the location chosen. I understand that; if I OPT NOT to participate at that time, George Online LLC, and Household of Faith is not obligated to accommodate me now or in the near / far future in WATER BAPTISM / BAPTISM BY WATER.

It is understood and agreed that this Agreement is made and received in full and complete understanding, settlement, and satisfaction of the causes of action, claims and demands mentioned herein; that this Release contains the entire Agreement between the parties; and that the terms of this Agreement are contractual and not merely a recital. Furthermore, this Release shall be binding upon the undersigned, and his/her respective heirs, executors, administrators, personal representatives, successors, and assigns.

This Release shall be subject to and governed by the laws of the State of ARKANSAS, This Release has been read and fully understood by the undersigned.
